Agreement for Remote Access to the PH&S Network
It is the goal of Providence Health & Services (PH&S) to provide the patients we serve with the best care possible. In order to do this PH&S will utilize a variety of technologies available in the market to enhance the quality of care through improved communications with the various employees, business partners, physicians and other healthcare providers who utilize our facilities. PH&S intends to provide access to these services in a manner consistent with the goals and objectives of the PH&S organization, consistent with all legal and regulatory requirements, and in as fair and equitable a manner as possible.
Technology is constantly evolving, and is subject to a variety of influences that may affect the ability of our organization to consistently provide these services at any given moment in time. The specific technologies offered at any given time are subject to a variety of factors, and may change without notice. This agreement is intended to define the current terms and conditions for use of these services.
Data residing within all computer systems, including personal computers, intelligent workstations, networks, servers, personal digital assistants and any storage media are the sole property of PH&S. PH&S reserves the right to review, audit, intercept, access and disclose all data residing within these systems at any time without notification. Anyone accessing data owned by PH&S is subject to the policies and regulations set forth in this and other relevant documents. Permission to access data may be granted to employees, business partners, contractors, physicians and their staff under the following conditions:
Providence Health & Services Responsibilities:
- PH&S may provide connection to the PH&S Network via a variety of technologies. The performance and reliability of these technologies may vary, and may not entirely be under the control of PH&S. Use of third party services (Telecom providers, Internet Service Providers (ISP's), etc.) may be utilized. PH&S will to the best of its abilities attempt to utilize reliable partners in providing connectivity to PH&S, but can not be held responsible for the performance of these third party services.
- PH&S will make a good faith effort to ensure the availability and reliability of all PH&S computer systems but PH&S is not liable for any business loss due to these systems being unavailable.
- The PH&S electronic mail (e-mail) system is intended for business use only with occasional personal use permitted with discretion. PH&S will attempt to ensure the confidentially of all e-mail messages but PH&S reserves the right to review e-mail messages for inappropriate use, to re-address messages received with inappropriate addressing, or to respond to legal requests. PH&S e-mail servers may be backed up on a periodic basis resulting in long term storage of e-mail messages.
- PH&S may provide Internet access to the remote users. Such access is for business use only with occasional personal use permitted with discretion. Personal use of the Internet may never include pornographic or threatening sites. PH&S monitors use of the Internet and inappropriate use may result in suspension and/or termination of Internet privileges.
- PH&S will attempt to protect its computer systems and e-mail messages from computer viruses but PH&S is not responsible for any business loss due to computer viruses.
- Use of the PH&S Network is for the convenience of PH&S employees, business partners, physicians and their office staff. PH&S reserves the right to terminate network access with 30 days written notice.
Remote Access User Responsibilities:
- The remote user is responsible for providing their own personal computer(s) (PCs) and for configuration of those PCs. The remote user is responsible for any intraoffice networking and configuration changes for PCs used to connect to the Providence Health & Services (PH&S) Network. If necessary, the remote user will be responsible for providing cabling installed in walls in order to connect to the PH&S Network. If desired, remote users will be responsible for providing hubs and routers in their office to provide for intraoffice networking or for the connection to PH&S. Specifications for connecting to the PH&S network may be obtained by contacting the Information Systems department at PH&S.
- Each remote user hereby agrees to comply with the PH&S Patient Confidentiality Agreement. Patient care providers may access records for those patients for whom they are caring. Use of patient results review will be monitored by PH&S and inappropriate use will result in disciplinary action up to and including immediate termination of computer privileges, loss of privileges for providing care in the hospital, and possible civil and criminal penalties.
- Each remote user will be responsible for protecting the PH&S network from unauthorized use and entry that might occur from their access device(s). Network use will be monitored and inappropriate use of the PH&S Network may result in loss of network privileges. This precaution applies to all vendors working on behalf of the remote user.
- Although PH&S will take all necessary precautions to maintain the security of the PH&S network, each remote user will be responsible for protecting themselves from unauthorized use or entry into their computers and/or private network.
- Each user is responsible for the use of PH&S computer systems under his or her login and password. All users are reminded that they may not share their password with anyone else and they may not post their password for their own convenience or the convenience of other users. A login and password to PH&S computer systems is treated with the force and authority of an electronic signature. Each user of PH&S computer systems needs to have his or her own unique login and password.
If there is reason to believe the confidentiality of a user identification code has been breached, PH&S Information Systems will be contacted immediately for termination and re-issue of codes. Logon ID's, access codes and passwords will be deleted from appropriate systems upon user termination or administrative request. - Use of the PH&S network and computer resources will give the user access to knowledge about PH&S business and patient care practices, which the user may use in the care of patients. No user is to share such information with outside parties. Information gained through the use of an individual's access code is confidential and proprietary.
- PH&S Network Services are for the use of PH&S employees, business partners, physicians, and their staff who utilize PH&S facilities. Remote users are not authorized to provide these services to anyone else, including any vendors which may assist the remote user in connecting to the PH&S network.
- All software shall be used in accordance with its license agreement. Unauthorized use and possession of, duplication of, and installation of software is a breach of company policy, and may result in loss of access rights and other legal action.
- Internet access may be provided for use of specific functions via the PH&S network. Those with access are expected to use judgement and discretion which will not conflict with the organization's core values. The Internet contains information and material not appropriate to the work environment at PH&S, which PH&S is not responsible for. Access must be limited to, and within the scope of, PH&S's business practices which is governed by the same general rules that apply to appropriate use of PH&S property. Use of the Internet via the PH&S network is a privilege, not a right, and may be revoked at any time for inappropriate conduct. Internet use though PH&S services is not a private form of
communication, therefore, PH&S reserves the right to monitor a user's Internet utilization
at any time without further notice to the individual. Users must adhere to PH&S's policies on confidentiality and the release of information.
The Internet may not be used to transmit confidential or sensitive material, unless appropriate encryption and security mechanisms have been verified and approved by I.S. in advance.
NOTE: Once your account has been established, you are free to re-install the software on the same computer or install it on additional workstations without submitting an additional request.
